The reason this episode is worth summarising is that he refused the premise. Asked what makes clinic SEO a specialism, he said the ranking mechanics barely differ from a plumber’s. That answer is more useful than the one the question was fishing for, because it relocates the work: if the mechanics are portable, the value is in the copy layer, the compliance layer, and two unglamorous items almost nobody finishes.
The diagnostic, in two items
Most onboarding processes are a deck. Isaac’s opening pass is two things, and both are close to free.
The Google Business Profile. Not because it is exotic. Because every clinic filled it in during opening week and has not opened it since. Categories are wrong or incomplete, and the services list is frequently empty. Two fields, and they decide whether the business appears in the map pack at all.
Conversion tracking. He is upfront that it adds no traffic whatsoever, and puts it second anyway. Without it, every budget conversation for the next five years is a guess.

Where the specialism actually lives
In what a business is permitted to say about itself. Comparative claims are commonly prohibited. Words like specialized and expert are restricted in many healthcare jurisdictions. And the variation is not national, it is state by state and province by province, so copy that cleared review in one market can create a real compliance problem in the next.
There is also an advertising floor. Google will not permit ads on the more speculative services at all, which pushes that revenue into organic and into channels that are not search. That constraint, not the ranking algorithm, is what makes healthcare marketing a specialism.
The channel an SEO named as most underrated
Showing up in person. Sponsor the youth team, work a booth at the marathon, tape some ankles.
Which lines up with a play we run from the other direction. A community cleanup is a real event, and a real event harvests an unusual amount of digital surface: Eventbrite carries an API into ChatGPT, Google runs a dedicated SERP view for events, and a local subreddit that would normally reject anything promotional will accept a genuine community event. Run with the sponsor brand in the title and without it, the version carrying the brand earned more upvotes.
The leak nobody instruments
Everyone audits the contact form. Isaac pointed at the phone.

For a local business the phone is the primary conversion path, and it is the least instrumented one. Call tracking is usually absent where form tracking is present, so the biggest leak in the funnel is also the one nobody is measuring. He will not put an AI on the front desk to solve it either, and was explicit that the technology is not good enough yet.
The framework worth keeping
Google is moving from curation to selection.
Ten blue links is curation: the searcher gets a list and does the choosing. An AI answer is selection: it does the choosing and hands over one result. That reframes what a local business is optimising for, and it is the argument for why the business profile keeps gaining weight while the website loses a little. If you are being selected rather than listed, the structured record the engine trusts about you matters more than the page you wrote about yourself.
Worth reading next to what most location pages get wrong, because most of them are still written for the curation era.
